This Month's Theme isSoftware FMEANext month's theme will be 'hazard analysis'Every month in FMEA Corner, join Carl Carlson, a noted expert inthe field of FMEAs and facilitation, as he addresses a differentFMEA theme (based on his book Effective FMEAs) and alsoanswers your questions.Questions and answers are a great way to learn about FMEAs, for bothexperienced and less experienced FMEA practitioners. Please feel free toask any question about any aspect of FMEAs. Send your questions to,and your contact information will be kept anonymous. All questions willbe answered, even if they are not featured in the FMEA Corner. A P-Diagram is an optionalstep when preparing for a System or Subsystem FMEA.

It is mostuseful when the item under analysis is a complex system with manysystem interactions, operating conditions and design parameters, andthe team will benefit from seeing these elements visually. It is atime intensive step, but can provide great value in understandingand controlling the system, and identifying input to the FMEA.P-Diagram is at the Item level, not at the Function level.

In otherwords, there is one P-Diagram for a given system or subsystem, not foreach function in the system or subsystem. The information in the IdealResponse block of the P-Diagram becomes input to the Functiondescriptions in the corresponding FMEA.In answer to your second question, the System block represents theentire system. As covered above, the information in the Ideal Responseblock of the P-Diagram becomes input to the Function descriptions in thecorresponding FMEA.

Following the creation of the project work group, AIAG solicited additional industry feedback – specifically from Tier 2 and lower companies – via a Voice of the Customer survey, which showed a strong international interest in replacing the previous Core Tools “spreadsheets on a CD” product with a cloud-based application.Tier 2 and lower suppliers voiced a critical need for support in successfully executing the Core Tools during their new product and process development projects. They called for a robust but flexible solution: a tool that provides the required forms, but also allows for customization to support their unique needs.
